From supporting people in crisis to helping people take the final steps towards independence and employment, we make a difference to 10,000 lives every year across all 32 boroughs.As the Referral & Assessment Lead Worker, you will be the first contact our referrers have with our service. You will be overseeing the referrals process, the allocation of clients for assessment as well as the liaising with professionals who refer to our service. You will also hold a caseload of clients who have varying support needs that are linked to tenancy sustainment and do so within a recovery-focused framework. You will need to have a strong understanding of Support Planning and Risk Assessing, as well as a keen understanding of the challenges our clients face when accessing services.To be successful for the role, you will have:A level of experience and understanding of provision of services to people within the areas of offending, mental health, homeless people with complex needs & a history of substance useDemonstrable level of experience, practice and understanding of the principles of risk and needs assessment, safeguarding, planning, goal setting, and reviewing of client supportThe ability to engage with a wide range of professionals, some of whom may have very different approaches to problem solving through a multi-agency approach, and advocate to agencies on behalf of the service and its clientsTo provide information and advice to clients about the options available to them, with the aim of empowering clients to make informed decisionsTo communicate confidently and effectively, verbally and in writing to professionals as well as clientsYou will also need to be adaptive, supportive and innovative in your approach to client support and collaborative work.
