Job Description : Evolve Support Services is a semi-independent living service for care leavers aged 16-18. Our aim is to support young people to develop their independent living skills. We are looking for committed and dedicated staff who share our passion for delivering a quality and holistic approach to our Young people.Our young people come from diverse backgrounds and they are often vulnerable or display challenging behaviour. Evolve is committed to improve the quality of life’ for society’s most vulnerable young people. Evolve strive to deliver an optimum service to help young people achieve their full potential and lead a fulfilled life as they transition into adulthood.The role of the Support Worker is to provide support for service users in preparation for independent living in line with their individual support plans. This will be done through our semi-independent living provision where there will be 24 hour staffing. You will be required to work with service users in supporting them to develop on their life skills. You will be expected to provide practical advice, emotional support, and assistance with the ultimate aim of enabling young people to lead a dignified and comfortable life in the community.Working hoursThis is a part time role 16.5 hour per week contract (minimum), with the sleep-in being in addition to your contracted hours.Key Duties and Responsibilities To fully understand the aims, objectives and ethos of Evolve Support Services and to reflect these in your daily working lifeTo work collaboratively with multi disciplinary professionals , attending meeting when required.To work in partnership with parents and the allocated social worker/placing authority.To ensure that all policies and procedures of Evolve Support Services are adhered to.Experience of report writing and record keeping.To ensure that regular opportunities are given for service users to feedback on the service and for consultation and to ensure that this information is used to improve the service.To keep thorough, confidential and up to date service user records for the purpose of effective and efficient service user working, using computer systems.To maintain the cleanliness of the premises.To be involved in the assessment of service user’s progression.To attend regular team meetings, training and supervisions to help your own professional development.To be responsible for the welfare of young people and their emotional well-being.To assist in enabling young people to take responsibilities, where possible, for their own welfare.To assist, if necessary, with the practicalities of everyday living, e.g. shopping, budgeting, cooking, travelling, socialising, reporting incidents and any other occurrence etc.To assist in the young persons’ social, recreational, educational and employment activities.To participate in assessment, planning and reviews of young people.To ensure that emergencies are responded to at any time as authorised by the Manager.To keep abreast of good practice in order to develop skills, knowledge and experience.To be open to systematic personal review and improvement through reflective practice;To report to management, or other appropriate person, malpractices or evidence which may suggest it.Able to work a flexible working pattern including evenings, weekends and/or sleep in duties as and when required.Company benefitsCompetitive rates of pay.Pension with company contribution.Access to well being /counselling support.Excellent training opportunities.EssentialPrevious experience working within a children’s residential setting or a supported living environment .Computer literateGood interpersonal skillsHave or working towards NVQ Level 3 in Health and Social Care.Full Driving LicenceThis post is subject to a Disclosure and Barring Service (DBS) check at an enhanced level.We actively encourage applications from all backgrounds to help us to provide the best possible experience for the young people we support and to make Evolve a great place to work .Job Types: Part-time, PermanentPart-time hours: 16.5 per weekSalary: £10.75 per hourBenefits:Casual dressCompany pensionOn-site parkingSick payWellness programmesExperience:Support Environment: 1 year (preferred)Licence/Certification:NVQ Level 2 Health & Social Care (preferred)Application deadline: 27/03/2022Reference ID: ESS2022Expected start date: 18/04/2022
