Job Description : Care Coordinator, a role where you make a real difference in someone’s life

We are looking for a highly motivated individual to join our team to manage the organisation and delivery of care. You will manage a team of care assistants to support our service users in their own home by providing person centered care. This role is more than just rota management. You will form part of our operational team who ensure that we provide outstanding quality care to service users.

Specifications of the role:

Rostering and daily management of Care Staff who are based in the community
Working with multi-disciplinary teams and professional bodies to ensure outstanding care is delivered and to positively promote our services.
Complete quality calls with care staff and service users
Provide reports to senior management as and when required.
Provide support to our teams as part of a rostered-on call system.
Manage referrals from commissioners and local authority
Ensure full capacity is utilised
