South and West Community Mental Health Team for Older People has a rare vacancy for a Care Coordinator. Based in the busy city of Bradford we cover the South and West areas of the city. We are a team which works both office based and in an agile manner which means after induction you will be able to start and end your working day at home or in the office.
We are looking to widen our team skills and experience, and would welcome applicants from allied health professionals with the relevant qualifications and experience, as well as registered mental health nurses.
We care for people over the age of 65 with any mental health problem and those of any age with dementia. We work closely with our inpatient wards to prevent admissions.Shortlisting date: 29th March 2022Interview date: provisionally 8th April 2022
