Reporting to: Sr. Medical Social Worker
Qualification: Graduate
Experience: Fresher
Job Responsibilities:
Make regular rounds in the patient area, visit patients and assess their needs within 24 hours
Participate in Interdisciplinary team rounds which needs to be completed within 48 hrs.
Provide pre-operative and post-operative counselling. / discharge counseling /DAMA counseling
Encourage interaction between new and old patients of similar problems to create a support group.
Render grief counseling / psycho-social , emotional, moral support to patient/familiy, address end of life issues.
Coordinate Governmental and non-governmental organisations to procure relevant services (Including financial assistance) to patients.
Coordinate between doctor and patient/patient family and facilitate proper communication.
Facilitate interaction between the medical superintendent and the family In case family has any medical issues.
Gets and understanding on the patients progress with inputs from the treating team according to measurable goals described in treatment and care plan and clarifies the same to the patient family.
Arrange for post-operative rehabilitation services. (Make arrangements for material help / prosthetic help to the patients).
Interpret the nature of disease and its implications to the patient and his family members.
Support and bring environmental modifications as and when necessary in order to enable the patient benefit from treatment.
Interpret the patient’s psycho-social problems to the patient’s family to help them accept the patient as she/he is.
To enable patients with similar problems to come together to feel less isolated.
Educate patients and their family members regarding diseases and their treatment.
Any other jobs assigned by superior due to exigencies of work
