We are looking to recruit a full-time Senior Field Care Supervisor to deliver high-quality person-centred care and supervise a team of community facilitators across our Hertfordshire Branch.

You will work closely with the head of quality and compliance, managing team members working in the community, supporting care workers and service users with face-to-face reviews, supervisions, and daily support.

The role:

You will be supporting the Field Care Supervisor or our community facilitators within the community acting as by completing assessments and audits, developing relevant care plans, mentoring, coaching, and supporting community facilitators within the designated area. Your role will also involve the following –

Ensure all compliance is kept up to date and assign to the complete all supervisions, observations and reviews required.
Manage assessments of service users in an acute and home setting within your area.
Ensure all 6 monthly service user reviews are completed and review the standard of submission within your area.
Ensure care plans are up to date including care plan formats are safe, sufficient and in place within in your area.
Attend new referrals and advice service users on the different options of care available to them.
Seek feedback and surveys and check satisfaction from service users.
Oversee onboarding of our new recruits ensuring new recruits are signed off as safe and complaint.

Specifications of the role:

Support a team of Field Care Supervisors / community facilitators working in the community.
Carry out regular observations and supervisions of care workers within your area.
Carry out new assessments and create care plans for new service users.
Keep all the team aware and up to date on any changes to service user’s reviews.
Cover care worker absence, sickness, holidays and provide emergency cover.
Support service uses to achieve maximum independence.
Ensure adequate knowledge and comply with risk assessments.
Record complaints, incidents and accidents as per the reporting policy, keeping all parties involved up to date.
Complete weekly reports.
Complete audit reviews of compliance targets and process.

Essential Experience:

Must have a minimum of 2 years of experience within a Care setting.
Must be flexible on the hours you are available to work

Desirable Experience:

Hold a minimum of a level 3 Diploma or equivalent within Health and Social Care and be willing to undertake further training and qualifications.
Management or Supervisory experience.
Excellent Customer service skills.
Ability to work at a fast pace and adapt to change quickly.
