A full time Band 6 stroke nurse practitioner position has arisen on C4, a hyper acute stroke unit based at Queens Medical Centre. We are seeking an enthusiastic and highly motivated individual with excellent leadership skills to support the team.

Main duties of the job
Hyperacute Stroke Services have an exciting opportunity to join their experienced team of Stroke Nurse Practitioners. The role is based within the Emergency Department, providing emergency care to anyone admitted to hospital with a stroke. The role requires you to work closely with the ED, mechanical thrombectomy and stroke research teams to provide excellent care and improved quality outcomes for all stroke patients.

Detailed job description and main responsibilities
JOB SUMMARY
Work as an autonomous practitioner responsible for the triage and initial assessment, examination, initiation of treatments and investigations for suspected stroke patients adhering to protocols.
Implement, co-ordinate, review and evaluate care in collaboration with medical colleagues
Be responsible for the co-ordination of assessment and appropriate clinical transfer of patients with a suspected stroke
Coordinator and manage the shift on the hyper acute stroke unit
Be responsible for assisting with data collection to input to the Sentinel Stroke National Audit Programme (SSNAP), ensuring the patients receive best possible care in accordance to the Royal College of Physicians (RCP) Acute Stroke Management Guidelines
Work with senior stroke clinicians to review, evaluate and develop & implement plans to improve SSNAP data
To take senior management role in the absence of ward managers and other seniors across the stroke service
Ensuring that accurate and timely information is given to the patient and/or their relatives
KEY JOB RESPONSIBILITIES
Acting as an advanced practitioner and professional role model, demonstrate advanced clinical skills, promote clinical excellence and ensure that an optimum standard of nursing care is delivered from admission to transfer / discharge.
Adhere to professional codes of conduct at all times
Assist medical staff in assessing emergency patients, arranging for specific tests (blood, radiology etc.) and acting on them.
Triage and monitor all emergency patients with a suspected diagnosis of Stroke and act to ensure maximum standards of care are given.
Perform assessment and physical examination of emergency patients, initiate appropriate treatments and investigations.
Act as source of expert knowledge around thrombolysis and mechanical thrombolysis
Be proficient in the administration and management of patients receiving thrombolysis
Take responsibility for co-ordinating emergency out of hours stroke patient management in relation to thrombolysis
Assist in the collection of data to prove adherence to stroke protocols and performance against national stroke targets.
Capable of requesting diagnostic investigations as appropriate, and acting on the results.
Perform expanded practical clinical roles specific to the Stroke service for example cannulation, venepuncture, swallowing assessments, ECG recording and interpretation, CT scan requests & pathology requests
Ensure timely and appropriate transfer and/or discharge of patients from ward area, providing a seamless patient pathway.
Liaise with patents and relatives to ensure appropriate and informed care choices are enabled
