Job Description : An excellent opportunity has arisen to come and be part of our friendly Specialist Adult Community services. This is a Part Time Senior Occupational Therapist role to work with Post Covid Rehab, Pulmonary rehab service with MDT support.

You will be able to provide decisive and effective therapy input and can demonstrate leadership qualities, excellent communication skills and a flexible working option.

The duties of the service to which you are applying will involve potential to work in a care home setting. This means appointees are required to have the necessary certificate of proof they are either vaccinated or medically certified as exempt and legally able to work in such settings. In order to comply with our duty of care, we require our staff to have the COVID vaccine and we will ask for proof of vaccination during the recruitment process. If candidates are unable to meet the requirements then their application may not be progressed and a refusal to provide such evidence is grounds for an offer of employment to be refused.

To provide a highly skilled occupational therapy assessment, education, advice and treatment to adult patients referred to post covid rehabilitation and support pulmonary rehabilitation and other community services if required and work to the standards set by BAOT and HCPC.

To work as part of the Triage Team to screen and assess referrals into the team, including gathering medical history from GP records/referrer and identifying if appropriate or not; establish priorities and assign to appropriate pathways.

To undertake patient management/treatment in a variety of settings utilising a range of methods to do this e.g. individually or group therapy/rehabilitation interventions, face to face, virtual and app-based technology (Telehealth, self-directed or Supported Self-Management App).

To manage a caseload of clients with moderate to complex needs with supervision and guidance as necessary from more senior clinical staff. To successfully monitor, evaluate and review occupational therapy treatment/interventions within a multi-disciplinary framework and to modify as necessary taking a key worker role as indicated.

The Post Covid MDT consists of Physiotherapist, Occupational therapist, Clinical Health Psychologist and rehabilitation Assistant.

Vaccination

The successful applicant will be an NHS employee and may therefore have contact with vulnerable service users. During the COVID-19 pandemic, it has been necessary to take significant steps to protect the health and safety of our staff, service users and those attending our sites. To provide assessment and treatment of Post covid patients and chronic respiratory conditions that are referred to the service for fatigue, Cognitive issues, breathlessness vocational rehab support.
To undertake patient management/treatment in a variety of settings utilising a range of methods to do this e.g. individually or group therapy/rehabilitation interventions, face to face, virtual and app-based technology (Telehealth, self-directed or Supported Self-Management App).
To lead on assessments and on boarding patients for the appropriateness of digital self management app for post covid rehab.
To use specialist knowledge to refer to other disciplines as appropriate.
To be able to case manage a number of people with diverse presentations, agreeing individual patient goals/expectations regarding interventions, frequency of contact, progress review including specialist medical input
To be professionally and legally responsible and accountable for all aspects of own work including the management of patients.
To manage a caseload of clients with moderate to complex needs with supervision and guidance as necessary from more senior clinical staff. To successfully monitor, evaluate and review occupational therapy treatment/interventions within a multi-disciplinary framework and to modify as necessary taking a key worker role as indicated.
To use knowledge of other multi-disciplinary roles and external agencies to ensure effective referral management and achieve best client care.
To undertake highly specialist assessment of patients, interpreting complex findings, to enable an appropriate treatment plan to be formulated, delivered and progressed for a wide range of complex conditions, using highly developed clinical reasoning and skilful therapeutic handling developed from specialist knowledge and using extensive practical experience to guide the process. This includes
Comprehensive subjective history,
Physical assessment screening e.g. sit to stand in one minute, active stand test,
Appropriate Patient Reported Outcome Measures (PROMS) for e.g. fatigue, breathlessness, cognition and mood.
Contribute to the development and dissemination of NWL-wide self-management offer and resources e.g. recorded webinars
To support and supervise junior Occupational Therapists, Rehabilitation Assistants, students on placement and locum therapists.
To assess, prescribe, issue, and instruct patients in the safe use of a variety of equipment including a range of functional aids, manual handling aids and mobility equipment.
To undertake assessment of need for postural support/seating and work with wheelchair services and related services.
To assess, diagnose and develop individual care/treatment plans for a range of conditions, some of which will be highly complex or will be for patients with complex clinical and social needs, and to be responsible for reassessing patients care as treatment progress and alter treatment programme as appropriate. To evaluate and develop treatment programmes.
To advise and teach patients, carers and other professionals to participate in occupational therapy and intervention plans for the individuals.
To ensure a high standard of clinical and professional practice.
To undertake specialist occupational therapy assessment and treatment as an autonomous practitioner.
