Kapoor Medical Relief Charitable Foundation The Gunvati Jagan Nath Kapoor Foundation supports programs in Health Care, Community Empowerment and Education with an intent to improve and sustain the standard of living and quality of life of all people with a focus on Women and Children. The Foundation guides on-treatment cancer patients in three hospitals within India, encouraging them to sustain treatment and reducing the abandonment rate. It also encourages women from underserved communities to make informed choices about their health by conducting ‘Cancer Awareness Talks/Camps’ and talks on ‘Maternal Health’. The young and bright minds of the ‘Institute of Chemical Technology’ and ‘G.N. Khalsa College’ in Mumbai were supported. About the Program & Role Description The Foundation initiated the Patient Guidance Program in November 2013. The goal is to help cancer patients access the resources they need while undergoing treatment and decreasing treatment abandonment rate. The services being provided are: documentation assistance, financial guidance and aid, accommodation guidance and aid, cancer education, individual support sessions, patient advocacy and directing patients to hospital related resources. Currently our Patience Guidance Program is operational in four hospitals namely ‘Tata Memorial Hospital’ in Mumbai, Tata ACTREC in Kharghar, ‘Sant Guru Ramdas Charitable Hospital’ (SGRD) and ‘Guru Nanak Dev Hospital’ (GMC) in Amritsar, Punjab. Job Title: Patient Guidance Assistant/ Coordinator Location: SGRD hospital, Amritsar Type of contract: Consultant (Part Time / Full Time) Reporting to: Project Manager Job Purpose: The purpose of this role is to coordinate with various stakeholders and execute the project in the hospital (serving cancer patients) in order to deliver high quality and effective output. Job responsibilities: Will be responsible to guide & counsel the patients and their caregivers who visit the hospital for treatment. Will be responsible to conduct, individual and group chemo education/psychosocial support sessions for patients and their caregivers. Conduct need assessment and/or any survey in the departments as per the instructions of the Lead/Head Will be responsible to network and work closely with the doctors, volunteers and other stakeholders of the hospital. Will be responsible to update and maintain data. Will be responsible to maintain reports and other documents and submit the same to the management on regular basis. Will work on other tasks assigned by the management. Essential Criteria: Candidates must possess a Graduate or Post Graduate degree in Psychology/ Sociology, Human Development, Social Work and other related disciplines. All candidates must have minimum of 1 to 3 years of work experience in the development sector preferably in areas of health, wherein the candidate has shown capacity in direct community work and coordination with beneficiaries and other stakeholders. capacity building, and direct implementation of the programmes.
