Job Description : o Review of the drug order is done by PAT.o Auditing all the in-patient indents for drug name, strength, formulation, dosage and duration of therapy, therapeutic duplication and to check for possible drug interaction, if any.o At the time of indent checking, any type of confusion or query is to be confirmed by phone with doctors, sisters and ward pharmacists,o Narcotic prescription is cross checked by clinical pharmacist as per narcotic policy.o Daily report of Restricted antibiotic to be sentIn case of any non formulary drugs are indented inform to the concerned doctor and substitute it with the drugs given in the formulary.Medication Tracer rounds e to be conducted by Prescription Audit team as required, and report to be generated.Responsible for checking drug appropriateness for Transplant patients.Responsible for checking the checklist of drugs in imprest stock, crash cart, disaster kits, Code blue bag and ambulance regularly.Monitoring of adverse drug reaction in hospitalJob Types: Full-time, Regular / PermanentSalary: From ₹15,000.00 per monthSchedule:Rotational shiftEducation:Bachelor’s (Preferred)Speak with the employer+91-XXXXXXXXXX
