Function of the Care Coordinator:
It’s our vision to provide the best care for children and families in our community and it wouldn’t be possible without you! Our therapists and psychiatrists carry full caseloads so they don’t always have time to track everyone who hasn’t responded to their emails; so that’s where you come in. You’re responsible for following up on any care coordination that our providers need and making sure that documentation of said care coordination ends up in the client’s electronic medical record. Care coordination can be anything from helping the biological parents obtain a bus pass so they can make it to their appointments or just emailing back and forth with the client’s Case Manager regarding their therapy attendance. Our clients are involved in a complicated system so the requests will run the gamut. Using your creativity and ties to the community, you will help ensure our clients receive the care we’re known for!
Did you think we were done?! This job has a few more layers than that, but we’ll unfold them slowly as to not overwhelm you. Since you’re out in the community, interacting with our referral source, we expect you to be the eyes and ears for where we can improve. Where do you think we came up with this position? Why this job stinks:
Did you see above where you’ll be doing the care coordination for every therapist and psychiatrist? That should say enough but we’ll elaborate a little further. We have a lot of providers on staff so the requests will be coming at you full steam ahead; buckle your seatbelt!
Another layer: you’re also not stationed at one of our offices, so it can be difficult to feel like you are part of our company culture. Your main office will be at one of our referral source’s locations but you will be traveling to other referral source’s locations as well as our offices around town. We only have the best of the best when it comes to Office Managers and they regularly include traveling positions (Yay! You’re not the only one!) in office activities, which are plentiful!
Last layer: since this is a new position it will grow and change as time goes on. Constant changes and new developments can be frustrating but we hope you will see this as an opportunity to assist in the creation of this position. Your feedback will be paramount to shaping out the Case Management role works within the Company.

Job-Specific Duties and Responsibilities:

You will be working with Case Managers, CAP attorneys, CASA workers, PSR/BST workers, biological families, foster families, and anyone else working with one of our clients to coordinate care and to be the person who links them to community resources. That’s a lot of people to be in contact with!
You’ll most likely be in contact with county Case Managers the most. Build relationships with them so they know who you are. This will benefit not only the Company but it will make your life a lot easier since they’ll be more willing to answer phone calls and emails when they see it’s from you!
The best part about us is our multidisciplinary team, and you’re another layer to that team! Bring your knowledge of case management and your skills of linking clients to resources.
As a care coordinator, and part of case manager’s responsibilities, you will advocate for your clients and their families, and will link them to natural and community resources. Please be sure that you are aware of those resources, and any new resources that may be available.
As we’re sure you know, record keeping is extremely important so we expect you to keep records of our client’s progress during treatment, including everyone’s phone call and email.
We need someone to be advocating on our client’s behalf. Our therapists and psychiatrists will be your teammate in this, but since you’ll be contacting many other outside resources, we’ll need you to keep the bigger picture in mind and remind the team of previous goals.
Our clients often need assistance with linking to community resources, so work your magic and get them set up with what they’re needing! This can be anything from a bus pass to housing- we know you can do it all!
Our providers carry a caseload of clients so we’ll need you to monitor communication and follow up when a community partner does not respond in a timely fashion. This will require extreme organization skills since there could be dozens of emails sent per day.
Since you’re in touch with community partners we’ll need you to be communicating with us what feedback you’re receiving. If there’s something we can do differently, let us know! We can’t guarantee we’ll make a change every time, but within reason we’ll try to accommodate.
Each site has a daily huddle and a weekly clinical consultation meeting in addition to weekly supervision via skype. You’ll need to attend all of these meetings to report back on what care coordination you’ve been providing and to offer assistance if a provider has been having trouble contacting a community partner.
In some cases, you might need to be part of the client’s treatment planning process. Some clients may require more time than others so if that’s the case, you’ll probably have to be available to give updates surrounding treatment planning. If a goal was obtaining a bus pass and you help them do that, the therapist is going to need to know to mark that goal as completed!
Sometimes what our clients need most is someone to just listen to them. This can be a fine line to walk since you’re not a licensed clinician. You’ll need to know your boundaries, and know when listening turns into enabling, giving advice or even being therapeutic in nature.
When connecting clients to community resources you’re helping to teach them how to do this in the future. Keep them in the loop about what you’re doing, how you’re doing it, and who you’re contacting so if they’re stuck in the same situation again, they’ll know where to start.
As a Care Coordinator, you will be spending a lot of time within various families. In doing so, you become part of that family system. You will need to be aware of that and remember what your role is in a professional capacity. Setting boundaries with your clients is important. Your goal is to empower the clients and to teach them self-determination and self-reliance, not to enable them.

Assigned Reports

We’re going to need you to update your tracker daily with who you’re contacting and how often. This will help us to learn where the need is and to make sure you’re not being over worked!
There are also contact sheets you’ll need to fill out weekly that summarize your conversation with county Case Managers. This helps us to see where problem areas are so our Leadership team can work on resolving issues.

Qualifications:

At least a two-year degree in related field or equivalent combination of education and experience
Effective, efficient googling skills
Hold a conversation with a stranger for a period of time about any topic. Seriously.
Leads by example, team player attitude, and shares our values – integrity, flexibility, empathy, excellence
Knowledge of the DSM and basic mental health terms will make the job more interesting and make you more successful
Awareness, sensitivity to, and respect for cultural variation and the power of personal values and biases
Ability to remain calm and courteous in the face of rudeness and disrespect
Proficiency in computer skills, especially using Word, Excel, and customized databases- this is a must!
Has computer, email, and internet access from home or other off-site locations
Demonstrates presentation and interviewing skills with ease
Inability to crumble under pressure or when speaking publicly
Ability to predict the future. Not really but you have to be able to identify warning signs for when something is likely to go wrong and then generate and implement solutions.
Ability to consider multiple perspectives and alternate explanations when problems arise
Ability to identify pair problems with solutions along with alternative solutions when you don’t get your way
You have to be able to prioritize and get things done
