Job Description : General Description:
The Case Manager I is responsible for providing guidance to all behavioral health patient’s to achieve their highest level of independence. Targeted clinical case management is a collaborative process which assesses, plans, implements, coordinates, monitors, and evaluates the selection of services to optimally meet each individual patients’ needs.
Roles & Responsibilities:

Assesses patients referred to the program to include any problems, needs, or barriers identified in the following areas: Housing, Employment, Education/Training, Financial, Sobriety Support/Addiction Treatment, Legal, Family/Social, and Medical/Psychiatric/Behavioral
Coordinates care with COMC personnel and other community partners on behalf of clients to for behavioral services.
Assists in patient intake process and compiles patient files ensuring all program eligibility documentation is complete
Collaborates with patient’s to complete Individual Action Plans (IAP)
Collaborates with patient’s to develop social skills, interests, and leisure time activities to provide a sense of participation and personal growth, including opportunities for age appropriate, culturally appropriate daytime and evening activities
Collaborates with patient’s to find and make use of appropriate employment opportunities, vocational rehabilitation services, or supported work environments where appropriate
Collaborates with patients who need specula living arrangements in locating a rehabilitative or supportive housing arrangement. The choices should be broad enough to allow each consumer an opportunity to live in an atmosphere offering the degree of support necessary, while also providing incentives and encouragement for consumers to assume increasing responsibilities for their lives
Offer support, assistance, consultation, and education to families, friends, landlords, employers, community agencies, and others who come into contact with consumers, in order to maximize benefits and minimize problems associated with the presence of these persons in the community
Identify and work with potential natural support systems such as neighborhood networks, churches, and community organizations
Make full utilization of the patients’ support systems in order to encourage treatment program engagement
Reach out to program eligible patient’s to inform and educate them about available services
Maintains confidential patient files in a professional and timely manner ensuring all forms are the most recently revised versions and in compliance with organization standards; all required documents are present, complete, signed by the patient and staff as applicable; and documents are placed in the correct order according to organization standards
Meets with patients as specified by program requirements and documents progress notes in designated format
Reviews and documents updates for IAPs
Completes timely discharge summaries
Assists in quality management activities, data collection, and preparing reports
Facilitates psychosocial/life skills groups/classes
Maintains confidentiality in compliance with the Health Insurance Portability and Accountability Act (HIPAA) standards
Completes accurate and timely data entry into the AWARDS system as applicable and other required internal and external databases
Documents client referrals for services
Attends scheduled team meetings and trainings
Maintains and submits time sheets, mileage logs, client expenses, and other required documentation accurately and in a timely fashion
Perform other duties, tasks, projects as assigned to meet changing business conditions, clinical demands and priorities

Work Experience (Minimum Requirements):

1 year experience in mental health or human services
1 year experience as a Case Manager/Worker or similar profession
1 year experience and knowledge of Psychotherapy
1 year experience and familiarity with behavioral health diagnostic techniques

Education/Certification/Licensure (Minimum Requirements):

High School Diploma (required)
Associates/ Bachelor’s Degree in Social Work, Psychology, Counseling, or other related social service field (preferred)
Possession of, or the ability to obtain an appropriate CPR Certificate.
COMC is a CMS provider and are obligated to follow CMS guidelines, as of 3.31.2022 in compliance with CMS Omnibus COVID-19 Health Care Staff Vaccination Interim Final Rule, all COMC employees, contractual providers, volunteers and students/interns are required to obtain vaccination against COVID-19 unless a medical or religious exemption request is approved.

Job Type: Full-time
Pay: $18.00 – $21.00 per hour
