Position Summary:
The Case Management Assistant will work in conjunction with case managers, nursing, physicians, ancillary hospital departments, patients and families to identify and assist in the coordination of post-hospital needs of all patients, infant through geriatrics, in order to promote quality, continuity of care and safety for the patient. The Case Management Assistant is responsible for assisting the Case Managers in carrying out physician orders regarding transfers, home health care, durable medical equipment and the specific discharge needs of the patient, placing patients in the most appropriate setting upon discharge. He/she will monitor utilization of resources, track any avoidable days identified, respond to insurance requests for clinical reviews in a timely manner utilizing Interqual criteria and follow review protocols for the department.

Qualifications
MINIMUM QUALIFICATIONS
Education:

Graduate of a licensed practical nursing program

Experience:

Minimum of 1 year case management experience in an acute care facility or 2 years minimum experience in a managed care environment or rehab facility

License/Certification:

Current Basic Cardiac Life Support required with 3 months of hire
Case Management Certification preferred

Special Skills:

Must have strong communication skills both written and verbal, computer skills and knowledge of Interqual criteria
