Youth Advocate Programs, Inc.
JOB TITLE Intake

I. POSITION SUMMARY – Position completes intakes for at-risks youth populations and mans referral database .
II. POSITION QUALIFICATIONS
A. EDUCATION: High School Diploma or GED required with commensurate experience required. Associates Degree preferred.
B. SPECIAL TRAINING/SKILLS (Licenses and Certifications):
Licenses and Certifications – n/a
Problem Solving Skills- Investigate and provide root cause analysis of problems, suggests alternatives, evaluate outcomes, and make formal recommendation for solutions
Communication Skills-Clearly and Consistently verbally communicate priorities, ensure timely exchange of verbal information between employees and/or departments, Verbally inform employees at all levels within the organization with need to know information, provide well- organized and clearly written information, utilize appropriate grammar, punctuation, and accuracy in written correspondence, manuals, or publications.
Math Skills – Perform accurate basic mathematical functions such as addition, subtraction, multiplication and division. Perform basic mathematical functions, percentages, ratios, etc…
Computer Knowledge – Word Processing software, spreadsheet software, Internet/E—mail, Database/Contact Management software, Presentation/Muti-media software, desktop publishing
C. EXPERIENCE/TRAINING: , Preferred experience working in youth services, community programs, or program development.
D. MENTAL DEMANDS TYPICAL OF THIS POSITION:
Ability to manage multiple projects/tasks, utilize creativity, maintain a high level of decision-making, ability to adapt to a constantly changing work environment, maintain a high level of record keeping/routine paperwork, provide close attention to detail, ability to travel, ability to work under pressure for results, establish own goals, meet frequent deadlines, work in a closely supervised environment, and work closely with others.
III. JOB RELATIONSHIPS
A. RESPONSIBLE TO: Typically reports to the Program Director, Program Coordinator
B. WORKERS SUPERVISED: Typically does not require the supervision of staff
IV. MAJOR JOB RESPONSIBILITIES/ESSENTIAL FUNCTIONS (list functions that can be measured)
1.
· Prepare intake paperwork for Program Director or Assistant Director.
· Call families to schedule intake within 24 hours of receipt of referral.
· Receive referring authority client referral and schedule intakes.
· Input service data by close of business after intake.
· Assist parents and clients in time of need while maintaining high client satisfaction; maintain detailed notes of all conversations in the appropriate client file.
· Communicate all information regarding clients and families to the Program Director or leadership within two hours of the communication.
· Maintain master client list by checking at least every two days and making appropriate corrections/additions.
-Accept referrals and contact in a timely manner
-Create and follow a contact logs including tracking referrals, documenting attempts, and follow up with missed or unscheduled intakes
-Conduct orientation to at-risks youth 8-18years old
-Assist youth in completing service plans and identifying needs through assigned referring authorities
-Input completed intakes into proper data entry systems including verification of documents, scanning and uploading to fulfill requirements
-Correspond with referring authorities to update status
2. Outcomes: Regularly submit program outcomes through Outcomes Management System.
§ Maintains internal and state (RA, county) tracking systems
3. The Support Center: Assist Program director and field staff with ensuring that the program is familiar with adhering to YAP’s business processes at the Support and Business Center.
§ Open communication with the program manager throughout the week
4. Documentation: Ensure that the program(s) is in compliance with federal, state, local and YAP documentation requirements.
· Create and maintain client files; file client paperwork by COB.
· Handle telephone calls and provide messages to appropriate staff no later than the close of the business day.
· Provide client paperwork for Advocate/TSS and update changes to client paperwork within 24 hours of change.
· Close out discharged clients within week of discharge.
· Handle incoming/outgoing correspondence, e-mails, and faxes.
5. Other Essential Functions of job but not limited to:
A. Perform other duties as assigned by Program leadership.
B. Adhere to all YAP, Inc.’s personnel policies, the YAP, Inc. Code of Conduct and the Integrity Compliance Plan.
C. Follow YAP, Inc.’s special incidents reporting procedures
D. Maintains a professional atmosphere and office space.
E. Attend meetings and trainings.
F. Obtain 20 hours of training per year
G. Attend all assigned trainings.
Job Type: Part-time
Pay: $15.00 per hour
