Job Description : Social Worker, or Case Worker, is responsible for managing multiple clients and their individual needs. Their duties include meeting with clients and their loved ones to discuss recent successes or setbacks to their situations, developing treatment plans for clients with mental illnesses or clients recovering from traumatic events and frequently communicating with clients to ensure their physical and mental health.
Job Responsibilities
Participates in patient care conferences
Participates in the development of the plan of care
Prepares and evaluates psychosocial assessments and development of social work service plans.
Provides clinical treatment and prevention of psychosocial dysfunction, disability, or impairment, including emotional and mental disorders.
Makes an initial evaluation visit to each patient for whom the physician orders home social services, and must devise a written plan of care for the physician’s approval. The social worker must review this plan of care as often as the severity of the patient’s condition requires, but at least every 60 days.
Prepares clinical and progress notes
Participates in discharging planning
Job Qualifications
A person who has a master’s or doctoral degree from a school of social work accredited by the Council on Social Work Education, and has one-year experience in a health care setting.
Three to Five (3-5) years of experience preferred
One (1) year of experience in home care, preferred
Ability to exercise initiative and independent judgment
Job Types: Full-time, Part-time
Pay: $75.00 – $100.00 per hour
