Position : Home Care Scheduler / Case Mgr
Job Description : The Scheduler/Case Manager works collaboratively with clients, families and Care Specialists to identify, coordinate, and provide individualized services to clients. They supervise the delivery of services to ensure that services provided are in accordance with CDPHE regulations, and to ensure client and family satisfaction. They collaborate closely with other members of the Client Services department, as well as other office staff members.
Essential Duties:
Displays professionalism, kindness, integrity, collaboration, and a can-do attitude in all interactions with employees, clients and family members
Demonstrates an understanding of CDPHE regulations and Agency policies and is able to effectively communicate these to employees, clients and families as necessary
Demonstrates a thorough knowledge of the needs and concerns of the clients to whom they are assigned as a Case Manager, and a general knowledge of the needs and concerns of all other Agency clients
Works closely with other Schedulers, Case Managers, Client Services Support Specialists, Intake Specialists, and Consultants on scheduling and other client-related issues
Supports staffing decisions made by colleagues
Demonstrates careful attention to detail especially when scheduling
Performs regular supervisory visits for clients and Care Specialists
Introduces new clients and new Care Specialists
Maintains accurate and complete client records, ensuring Care Plans are up to date
Completes and performs regular performance evaluations for Care Specialists
Coaches and encourages Care Specialists toward the goal of professional excellence
Collaborates with the Manager, and the Human Resources department to resolve employee discipline issues
Ensures incidents and complaints are promptly investigated, resolved, and accurately documented
Provides on-call support in the evening, on weekends, and on occasional holidays as assigned
Provides direct client support on an as needed basis, as directed
Tracks data regarding missed/cancelled shifts, hospitalizations, ER visits, and other details as requested
Demonstrates proficiency using Microsoft Office and is receptive to the use of technology to improve operations and overcome problems
Responds to new business calls as needed
Collaborates with other Agencies involved in the clients’ care
Performs miscellaneous clerical tasks associated with client services and scheduling
Demonstrates an understanding of HIPAA practices and is able to communicate these to clients, families and Care Specialists
Complies with all State Guidelines of Service
Other Duties:
Verifies the need for holiday care with clients and families and communicates holiday schedules with Care Specialists
Participates in Annual Training and other office events
Other duties, as assigned
Job Qualifications:
Must be at least 21 years of age and have a high school diploma or GED
Must be a mature, motivated person who is a team player
Must have proven verbal and written communication skills, the ability to work well under pressure, and a willingness to be flexible and adaptable as needed
Must enjoy working in a fast-paced environment, talking on the phone with employees and clients daily, be gifted at working with various personality types and be able prioritize and able to produce positive results
Must be able to follow verbal and written instructions
Must have excellent listening skills and the ability to ask probing questions politely
Must have good phone skills and excel at Customer Service
Must be organized
Must be self-directed and be able to work with minimal supervision
Must be courteous and able to maintain confidentiality
Must be competent using Microsoft Office and receptive to the use of technology to improve operations and overcome problems
Must have a positive attitude and exhibit patience and good judgment
Must have a valid driver’s license and the ability to maintain proof of current automobile insurance
Must have access to the internet outside of the office
Must be able to bend, stoop, and lift up to 25 lbs.
Working Environment:
This position is a 40 hour/week, non-exempt position, primarily working in the office
This position requires regular evening and weekend commitments to be on-call, and for occasional office events
This position requires occasional holiday on-call assignments
This position will involve regular driving to clients’ homes and occasionally to meetings
This is an administrative position but will require direct interaction with clients, families, employees and business associates
