Job Description : Job Description
JOB SUMMARY:
The Level I Medical Record Clerk performs clerical functions necessary to the operation of the Medical Record Department. To collect, maintain, and make available to authorized users timely, accurate, and complete patient health information. Prioritizes, locate, pull, electronically track, and sees to the appropriate transport of records requested by other departments/LHC staff members in accordance with established standards.

ESSENTIAL FUNCTIONS, DUTIES, AND RESPONSIBILITIES:
Scans paper documentation into electronic medical records.
Pick-up & distribute charts and loose sheet from all areas in the hospital and/or clinic settings.
Check-in charts.
Answer incoming calls to Medical Records Department and/or Clinic.
Route incoming mail and stamp outgoing mail.
Process walk-in requests and faxed requests for copies of medical records.
Pull and deliver charts for current in-house or surgery patients.
File charts accurately.
Sort and file loose sheets accurately and in accordance with established criteria for paper and electronic scanning.
Makes new patient charts and looks for missing charts.
Maintain Supply Cupboard and orders supplies when needed.
Distributes daily computer reports as appropriate.
Assists other medical record staff when required/requested.
Identify files for off-site storage.
Crossed trained to perform specific Clerk II job functions, duties and responsibilities. Demonstrates attention to detail, accuracy, and data integrity.
Performs other duties as assigned.

ADDITIONAL RESPONSIBILITIES:
Attends orientation and other compliance/privacy/security/educational sessions.
Maintains strict adherence to confidentiality and HIPAA requirements.
Participates in continuous quality improvement.
