POSITION PURPOSE
Reporting to the Director of Care/Supervisor of Care, and under the guidance and direction of the Registered Nursing staff, is responsible for providing care, support, assistance, and supervision to residents/clients in the Health Care Centre; promoting independence in activities of daily living, including restorative care, and working in collaboration with members of the client care team.

MAJOR RESPONSIBILITIES
Participates with other members of the Care Team in promoting the health, well being and independence of residents/clients.Performs basic care procedures which s/he has been prepared for through instruction and training by Registered Nursing staff.Promotes independence in Activities of Daily Living (ADL) by assisting with personal hygiene, dressing, transferring and toileting.Reports changes in residents’ physical, mental, and/or emotional condition to the Registered Nurse or designate.Immediately reports any problems or incidents involving residents and/or staff to the Registered Nurse in Charge.Receives verbal and/or written reports from outgoing shift members and documents incidents and conditions occurring on previous shift.Liaises and promotes positive relationships with the medical and facility staff, residents, families and visitors.Attends and actively participates in relevant training programs and team meetings.Ensures services provided to clients/families meet Regional customer service standards and Branch Code of Conduct standards.May occasionally be required to respond to emergency situations outside of normal business hours, as related to the Departmental Emergency Plan.

QUALIFICATIONS
Completion of a Personal Support Worker certificate program or approved equivalent combination of education and experience.Up to one (1) year experience or practicum placement with resident care in a long-term care facility or other geriatric environment.Satisfactory Police Vulnerable Sector Checks (PVSC).Valid Ontario Class “G” driver’s license and reliable vehicle for use on corporate business.Knowledge and demonstrated ability in corporate core competencies including customer focus, communication, collaboration and personal ownership.Demonstrated basic computer literacy utilizing MS Office software applications and the ability to receive and respond to emails.
