Job Description : *This is a remote position with the requirement to live in Illlinois.
Job Description
Position Summary:
The Care Coordinator will assist members in arranging visits to medical providers and specialists to ensure all health needs are met. This position will provide support, encouragement and education to members.
Principle Duties and Responsibilities:

Responsible for engaging plan members in improved self-management and lifestyle behaviors for improved wellness and health management, focusing on members who have documented gaps in care.
Perform outreach calls to members for assessment and program engagement.
Follow-up on inpatient and ER discharges.
Work with medical providers to develop customized care plans for members including goals and plan of action to improve overall wellness and improved clinical outcomes.
Coordinate care with other health-related vendors for optimal patient care.
Provide ongoing education and information to members as requested.
Assist members with community resources.
Other duties as assigned by manager.

Education:
Bachelor’s degree OR two years’ experience as a care coordinator.
Experience:

Experience working with multi-cultural and variable socioeconomic populations highly desired.

Knowledge and Skills:

Knowledge of specific disease and lifestyle related topics.
Excellent verbal and written communication skills.
Innovation to be open to new ideas, approaches and information.
Be able to effectively influence and encourage others.
Anticipate, recognize and meet member needs and offer help.
Composed, positive attitude, clear thinking and focused under pressure.
Extremely organized.
