Job Description : Job Description – Clinical Pharmacist US Health Care IndustryShould work as remote clinical pharmacist, who is able to perform medication reconciliation which involves comparing the medication prescribed with the medications patient is taking and should be able to make clinical decision based on comparison.Should be able to identify drug-drug interaction, duplication of therapy, dosing errors for the medications prescribed to the patient.Should be able review the clinical data and make appropriate decision regarding medications and patient’s condition.Should be thorough with the brand and generic medications (their indication, dosage, contraindication, category).Medication adherence: Ensuring that there are no gaps in patient’s therapy.Should be able to handle outgoing calls, faxes, and emails with pleasant and professional demeanor.Communicates on behalf of the prescribers with the pharmacies, patients, payors, pharma manufacturers.Should be able coordinate with the in-house clinicians or other staff members.Coordinates for patient financial assistance and support services, copay cards, funding assistance and manufacturer support programs.Completes prior authorization (PA) for all medications. A thorough review of the patient’s chart notes statement of Medical Necessity must be provided, which includes diagnosis, signs/symptoms of the disease, prior failed treatments, contraindications, laboratory tests and results and any other pertinent medical information for the patient’s case.Works independently on other projects as assigned.Performs other duties as assigned by management.SPECIALISED SKILLSKnowledge of medical terminology.Excellent verbal and written English communication skills.Ability to work in a busy, fast paced environment as a team player.Strong attention to detail and quality assurance.Exhibits strong work ethic and is open to learning new skills.TECHNICAL SKILLSExpert level proficiency with MS Office, Outlook, Excel, Word, and PowerPoint.Job Types: Full-time, Regular / PermanentSalary: ₹21,000.00 – ₹24,000.00 per monthBenefits:Health insuranceSchedule:Morning shiftNight shiftCOVID-19 considerations:WE follow all Covid 19 protocols, we provide free Masks, We clean our office three times and is being Sanitized every three hoursAbility to commute/relocate:S.
