Job Description : Work Location: Chicago Read Mental Health Center, 4200 North Oak Park Avenue Chicago, ILWork Hours 8:00 am – 4:00 pm (M-F)
Under general supervision, and as a participating member of a Treatment Team, performs professional social work services in the treatment of mentally ill patients. These services include (among others) assessment, treatment, writing/submitting court reports, performing general case management, discharge planning and where indicated acting as a qualified examiner. Endures compliance with Joint Commission, Central Management Services (CMS) and other agency standards and objectives. Serves on the Department and/or Hospital committees as necessary at Chicago-Read Mental Health Center.
MINIMUM REQUIREMENTS
Requires a master’s degree in social work from a recognized college or university supplemented by one year of professional social work experience in the evaluation and treatment/habilitation of mentally ill, developmentally disabled, or other disabled patients.
Requires extensive knowledge of the theories and assumptions underlying the social work field.
Requires a working knowledge of the employing agency’s rules and regulations covering the social work field.
