Job Description : JOB SUMMARY: The Care Coordinator II provides telephonic services to CHC Members and their families for purposes of, identifying members with special health needs and available resource assistance. Work with the member or the member’s legal representative to develop a service plan. Monitor members with special healthcare needs as per HHS guidelines. Makes SSI referrals to outside vendor and follow up on the eligibility process with CHC financial and eligibility departments according to established guidelines. Works closely with case managers, Team Leads and Manager of Care Management to coordinate care for members with special healthcare needs.
MINIMUM QUALIFICATIONS:
1. Education/Specialized Training/Licensure: Bachelor’s degree in Sociology, Social Work, or Psychology preferred.2. Work Experience (Years and Area): Three (3) years managed care customer service experience
SPECIAL REQUIREMENTS:1. Communication Skills:Above Average Verbal (Heavy Public Contract)
Bilingual Skills Required:
Languages: Bilingual (English and Spanish) strongly preferred
Job Type: Full-time
Pay: $18.60 – $23.00 per hour
