General Job Description
Care Coordinator is a unique clinic-based support services position. This position will work with their interdisciplinary clinic care team and will assist Medi-Cal Managed Care patients in establishing care with a primary care provider, participating in special group health education projects. Coordinate care transitions between local hospitals and our clinics. Will act as a liaison between FHCSD and contracted health plans, breaking down barriers to care through interpretation and transportation services.
Job Roles* Facilitates care transitions between hospitals and emergency departments for new and established patients and schedules follow-up appointments in a timely manner. Securely retrieves all relevant external correspondences and attaches to patient’s Electronic Health Record for provider review.
* Facilitates establishment of Managed Care patient’s medical home and care with a primary care provider within 90-120 days of insurance coverage date; Schedules at least 1.5 initial health assessment appointments per business day.-
* Identifies linguistic needs and learning style preferences of patients, documents need as appropriate and assists in scheduling interpretive services. Also identifies transportation needs and facilities transportation access when necessary.
* Performs other duties as assigned.
* Performs thorough follow-up on select referral orders from providers, maintaining timely and accurate documentation of all Care Coordinator efforts and outcomes.-
* Provides direct, non-medical Care Coordination assistance to all patients as needed. Assists clinic care team with questions/issues related to Medi-Cal Managed Care patients and assists patients in navigating healthcare services.
* Provides non-biased insurance assistance to Managed Care patients including but not limited to: modifying and/or selecting their Primary Care Provider or assigned clinic, assisting patients in filing appeals/grievances, communicating with County and Managed Care plan staff, and acting as a liaison between FHCSD and Managed Care plans.
Education/Certifications/Licenses/Registrations**
* Ability and means to travel as needed in a timely manner within San Diego County, to locations that may have limited access to public transportation; proof of liability and property damage insurance on vehicle used is required. DRIVER REQUIREMENTS: Age 21; Licensed for a minimum of 3 years; No violations and/or accidents within 3 years; No vehicle related suspensions/reinstatements; No DUI, reckless or felony Driving with 7 years
* At least 2 years experience providing direct customer service required.
* Bachelor’s degree in public health or social services-related fields OR 3 years of closely related healthcare experience and a high school diploma/GED required.
* Experience providing case management and/or patient advocacy preferred.
* Or equivalent combination of education and experience that provides the skills, knowledge and ability to perform the essential job duties, and which meets any required state or federal certification requirements.
Experience/Specialized skills (including Language)**
* Ability to work effectively with patients from diverse social, cultural, and economic backgrounds.
* Ability to work well in both a team-based environment and independently.
* Basic computer literacy (i.e., use system to manage and schedule appointments, access electronic medical record information).
* Bilingual in English/ Spanish required.
* Intermediate interpersonal and customer service skills.
* Intermediate written and verbal communication skills.
* Knowledge of community resources, health, and social service systems in San Diego County.
