Position : Emergency Shelter Case Manager – Center for Transforming Lives

Job Description : Center for Transforming Lives is currently seeking a full-time Emergency Shelter Case Manager to work Monday-Friday and occasional Saturday hours as needed for programming.
The Case Manager is responsible for providing trauma informed care to aid in the successful transition for both the residential and when transitioning into community housing. The Case Manager will also be responsible for developing relationships, providing competent leadership in the event of an emergency or crisis situation, coordinating services with local shelters, assessing participants for the program, and ensuring compliance with program expectations. In addition, the Case Manager will be responsible for assisting participants with obtaining the necessary resources to achieve service plan goals. The Case Manager position does entail light travel for participant needs and other program related activities.
