Apreva Hospice provides individually planned Hopsice care to meet the special needs of patients and their loved ones. The Apreva team is a family that takes the time to build, develop and foster relationships with patients and their families, and our goal is to assist each patient in achieving physical, emotional, and spiritual comfort.
This position is directly responsible for introducing and educating families to Apreva Hospice and providing explanation of benefits based on Medicare guidelines. You MUST be a team player, have a positive attitude, understanding of the hospice philosophy and superior time management skills. Pay commensurate with experience.
Job Duties/Responsibilities include (but are not limited to): .
1. Provides Hospice explanation of benefits to patient and families seeking hospice services.
2. Carries out social evaluations and plans intervention based on findings. Counsel’s patient and family/caregivers as needed in relationship to stress and other identified coping difficulties.
4. Maintains clinical records on all patients referred to Hospice.
5. Provides information and referral services for organization patients and family/caregivers regarding practical and environmental needs.
6. Serves as liaison between patients /family and caregivers/community.
7. Actively participates in quality assessment performance improvement teams and activities.
Position Qualifications:
*Master’s Degree in Social Work accredited by the Council on Social Work Education or BS degree in Social Work or Licensed Vocational Nurse in the State of California, or Chaplain.
*Minimum of one year’s experience in health care or hospice industry. Understands hospice philosophy, and issues of death/dying.
*Demonstrates good verbal and written communication and organizational skills.
*Must be a licensed driver with an automobile with required insurance coverage.
*Able to work independently and with other staff members to accomplish goals.
*Forms professional relationships easily, maintains professional boundaries.
*Strong interpersonal and customer service skills.
*Able to sit, stand, kneel, bend, reach and walk throughout the workday.
*Reliable transportation with required insurance coverage.
*Able to lift at least 15 lbs. and carry effectively so as able to perform the above listed functions.”Work Remotely
