Position : Intake Coordinator/Case Manager

Job Description : Job Title: Intake and Insurance Coordinator
Position Description: Facilitate, coordinate IV medication, and verify insurance authorization for patients while organizing workflow and appointments for IV infusion clinic.
Major Areas of Responsibility:

Verify insurance benefits and co-pays for all new patients starting IV therapy
Completing intake referrals while coordinate with provider, pharmacy, and hospital case manager
Speak with patients to ensure they understand their specific care plan and payment obligations

Primary Objective: To ensure patients the smooth transition of care from clinic/hospital to outpatient treatment. To ensure patients are aware of the payments they will be expected to make during their course of therapy.
Specific Responsibilities of the Job:

Following up daily with patients and case managers for smooth discharge transition
Keeping orders current and accurate
Notify pharmacy/infusion nurses/insurance coordinator of new orders and changes to existing orders.
Maintaining detailed notes/documentation on payment plans and conversations with patients.
Documenting all pertinent patient information into ECW and CPR

Requirements of the Job:

Maintain flexibility and help others as needed throughout the day
Communicate effectively with physicians, care coordinators, patients, and pharmacy staff
Document all orders, interventions, and conversations pertaining to patient care
Follow all rules/expectations outlined in employee handbook

Job Type: Full-time
Pay: $20.00 – $22.00 per hour
