JOB DESCRIPTION SUMMARY
The Intake Coordinator is responsible for the overall coordination of Agency referrals for Medicare and non-Medicare clients under the direct supervision of the Intake Supervisor. The Intake Coordinator is a resource person for referral sources and works closely with other members of the team to ensure adequate coordination and transition occurs to home health. The Intake Coordinator will be responsible to comply with the regulatory guidelines governing the home health admission process.
Clinical
1. Receive referrals from physicians, facilities and staff, as well as enter referrals in the EMR system and clearly identify who called in the referral.
2. Ensure all demographic information is accurate on the referral form based on eligibility (Medicare and non-Medicare requirements).
3. Enter Non-Admit information into EMR with details in coordination notes if no visit was made. Communicates Non-Admit status to referral sources and/or Marketing team along with the reason for non-admission.
4. May be required to participate in on-call rotation.
5. Follow up on automated eligibility alerts.
6. Communicates with the Billing Manager if the insurance carrier is not Medicare to ensure verification of benefits occurs. Communicate benefits information with referral source as necessary.
7. Scans referral paperwork into EMR timely to ensure that it is made available to field staff.
8. Communicate acceptance of referrals clearly with referral sources.
9. Dedicated to delivering excellent customer service
10. Review and follow up on entitlement verification issues.
11. Update the EMR patient data as deemed necessary
12. Collaborate with other members of the Home Care Team to ensure patients have a smooth transition onto Home Health Service.
POSITION QUALIFICATIONS
1. Must possess a Medical Assistance Certification and /or a CA LVN unrestricted license
2. Minimum of two years experience in the health care industry with one year experience in home health preferred
3. Experience with Intake responsibilities preferred
4. Must be computer proficient and able to work with different software systems.
5. Excellent Customer Service
6. This role requires full COVID-19 vaccination.
