Position : Intake Coordinator

Job Description : Receives and appropriately documents new referrals in a timely manner. Participates in coordinating care with Nursing Supervisors, patients/clients, and the interdisciplinary team. Provides effective communication to patients/clients, staff members, other health care professionals and referral sources. Responds to Agency and patient/client needs in a professional and creative manner. Demonstrates commitment, professional growth and competency. Participates in Agency-sponsored in-service training. Participates in the Agency’s QAPI program. Promotes the Agency philosophy and administrative policies to ensure quality of care.
Job Type: Full-time
Pay: $15.00 – $22.00 per hour

Medical Receptionists & Patient Registrars: 1 year (Required)
