Job Description : Patient Advocate
We are currently seeking a compassionate, experienced, and highly organized Patient Advocate to join our growing team of professionals. In this role, you will play an essential part in our ability to provide exceptional care by managing the individual care of our members. You will be responsible for helping members of the health plan to navigate and access services, identify and overcome barriers to care, provide primary health education and health status assessment, adopt healthy behaviors, and document any interactions with plan members. In addition, the PA will provide social support and advocate for member health needs to improve care and health outcomes.
PAs will work closely with plan members, care coordinators, the member services team, and health care providers to coordinate and improve patient care and ensure the triple aim of the best care at the lowest cost for the entire group/population served. PAs will primarily be working on the telephone from the main office in Kingwood but may, on occasion, need to work directly with identified high-risk members in the community.
The successful applicant must have a compassionate nature, be knowledgeable about health care practices, and provide exceptional customer service.
Responsibilities

Act as an advocate and liaison between the member, provider, health plan, and applicable service agencies
Effectively work with people from diverse backgrounds to identify and reduce cultural and socio-economic barriers to care
Provide culturally appropriate health education on chronic disease prevention, maternal and child health, physical activity
Help members develop health management plans and goals
Provide ongoing follow-up, essential motivational interviewing, and goal setting with members
Follow-up with members via phone calls
Help members in utilizing resources, including scheduling appointments, assisting with identifying services, identifying a care team, and preventing financial hardship
Assist members in accessing health-related services, including obtaining a primary care provider overcoming barriers to obtaining needed medical care and social services
Be knowledgeable about plan benefits and resources and provide referrals to patients as appropriate
As requested, attend regular staff meetings, conference calls, training, and other meetings
Manage an assigned caseload of members
Work collaboratively and effectively within the team
Document case activities, plan of care, and result while strictly adhering to the policies and procedures

Other duties as assigned
Requirements and Qualifications

Associates degree or above in a relevant field or comparable experience. Pertinent additional certifications are a plus
Experience in healthcare, mental health field, and/or health insurance plans
Experience as a patient advocate or other relevant role
Awareness of and commitment to all requirements, both legal and corporate
Solid computer skills, including Microsoft Office
Clerical skills with the ability to type 50 WMP
Familiarity with office equipment and phone systems
Excellent phone etiquette
Able to diffuse conflicts and disagreements, then find a workable solution
Exceptional communication skills, both written and verbal
Superior organizational and interpersonal skills
Compassionate and kind demeanor
Able to multitask and prioritize tasks
Able to act decisively and maintain discretion
Knowledge of medical terminology
Willingness to work in a dynamic environment

Pay: 36k to 42k Annually (Guaranteed Raises Annually)

Pay: $36,000.00 – $42,000.00 per year
