Job Description : We are seeking a Patient Coordinator to join our team! You will be the point of contact for all matters regarding our patients for the company.

BASIC PURPOSE:

To act responsibly with oversight of the scheduling of Direct Care Worker.
Assists with the direction and coordination of consumer care.
Assists with the oversight, audits, grievances
Assures consumer records are maintained appropriately and that all Direct Care Workers function, according to licensure, state, and federal law.

PRIMARY RESPONSIBILITIES:

Have a replacement attendant when a callout occurs.
Schedule Direct Care Worker to provide care in consumer homes or places of residence. -Provide Direct Care Workers with consumer-specific information before covering a shift.
Document appropriately. -Submit schedules and missed shift reports (documenting dates times, reasons for non-coverage, attendants called to cover) to Human Resources by email and as requested by the administration.
Have a tangible overview of the caseload.
Have an open and accessible dialogue with consumers and Direct Care Worker.
Provide direction, supervision, and consultation to the consumer regarding complaints, grievances, and incident reporting.
Report to Human Resources and administration any need for employee disciplinary action.
Serve as a resource to all Direct Care Workers regarding clinical oversight, Policies, or general information as needed.
Participates in program planning, identifying new program areas, establishing policies and procedures related to service delivery.
Provide oversight to the Direct Care Worker on the consumer’s plan of care.
Document all complaints and incidents and participate in the resolution/improvement process.
Advise, assist, and collaborate with other professionals on an as-needed basis to assess consumer situations, staff problems, and coordinate referrals.
Demonstrates basic keyboarding and word processing skills and can use agency software.
Assists in seeing that there is proper maintenance of consumer’s records including emergency contacts, phone numbers, family members, addresses
Conducts monthly communication with each consumer

Management:

Understand the need for staff productivity and effective utilization.
Assists in the investigation of consumer and customer concerns and complaints.

Leadership/Professionalism:

Maintain current knowledge of home care principles and practices.
Develop and maintain cooperative working relationships with other home care professionals in order to encourage the exchange of information and services and to upgrade the level of professionalism and quality of home care services.
Be able to exercise independent judgment, critically analyze situations, and make recommendations using appropriate problem-solving skills.
Able to manage competing demands for time and resources and independently prioritizes work responsibilities.
Able to function effectively as a member of a team.
Maintain confidentiality of consumer and agency information according to HIPAA guidelines.
Maintain an appearance that projects a professional image.
Maintain satisfactory attendance.
Able to resolve conflict situations.
Identifies and progresses toward meeting personal and professional goals.
Demonstrate awareness of their behavior and its effect on others and the agency’s productivity and effectiveness.
Performs other duties as assigned.
