Job Description : RESPONSIBILITIES:

Contact all website, fax, and email referrals
Assist with incoming referral calls
Collect all patient information needed to create their patient profile
Send appropriate consent forms to clients
Thoroughly document all communication
Connect clients with the appropriate department after gauging their needs
Responsible for knowing and communicating basic information about all our programs
Follow up with referrals as appropriate
Schedule level of care assessments as needed
Keep in touch with referral sources as appropriate

REQUIREMENTS:

Strong attention to detail
Strong organizational skills
Compassionate heart for those struggling
Ability to multitask
Effective interpersonal skills
