Job Description : As a Support Coordinator, you will assist individuals with intellectual and/or developmental disabilities to coordinate both paid and non-paid services and supports that will aid them in living a fulfilling life within their community. This includes working with the service user and their team to create an individualized service plan that clearly includes the persons aspirations and goals; and on-going monitoring of services and completion of service documentation to ensure that the individuals needs are being addressed and regulatory standards met.
