Job Description : JOB FUNCTION: The Intake Specialist is the individual that provides outreach, direct support services and assisting in the coordination and implementation of participant servicesDUTIES AND RESPONSIBILITIES: The Intake Specialist is the direct service provider who is responsible for but not limited to:

Provide community resources and referrals to meet the needs of our participants.
Screen program applicants for eligibility via telephone intake and/or on a walk-in basis and/or during outreach services towards program’s target populations (including street and shelter outreach).
Assist in the design and implementation of individual training and support programs for the participants.
Assist with completion and submission of applications for public and VA benefits for participants.
Assist program applicants and/or participants with collecting and submitting documents as needed.
Track screenings and participant contacts in an accurate and timely manner.
Administer assessments to determine eligibility and appropriate interventions.
Conduct follow along and follow up services to current and former participants.
Provide data entry and update of databases in a complete, accurate, and timely manner.
Maintain accurate and up to date participant filing.
Maintain positive communications and relationships with his/her supervisor, participants, employers, landlords, partnering agencies, involved members of families and other individuals in the community.
Complete in a timely manner all logs, reports and participant evaluations as required.
Provide direct service as needed and required.
Perform all job related duties as assigned.

SKILLS & EDUCATION REQUIREMENTS – An Intake Specialist must be 18 years of age or older with a minimum of a high school diploma or its equivalent. Must have a good understanding of military culture. Appropriate and professional dress is expected. This position requires a special aptitude for confidentiality, organizational skills and good interpersonal skills. The Intake Specialist must provide excellent customer service.
PHYSICAL REQUIREMENTS: The Intake Specialist may spend long periods of time in a sitting position (95% of the day). He/she will use a computer, which will involve repetitive finger, hand and wrist motion and being able to see the computer screen (95% of the day). Since the position calls for intake communication, the Intake Specialist must have the ability to talk and hear (100% of the day). Other physical exertion could involve walking, climbing stairs, lifting, stretching, bending, and etc. (10% of the day).
OTHER REQUIREMENTS – The Intake Specialist may occasionally need to make on-site visits to participants where they are (shelter, streets, day program, in-home, family’s home, motel, etc.), agencies, and employers. Often these on-site visits are at a distance from the office, where no regular means of public transportation exists. Overnight travel is rare, but may be required for training and as part of the program. An Intake Specialist may transport a participant as part of his/her job duties. A reliable means of transportation, valid California licensing and insurance is required.
He/she must successfully pass a screening for illicit drug use prior to employment.He/she must possess a record clear from past criminal activity and submit to a Department ofJustice Criminal activity background check.
