Position : Youth Team Leader

Job Description : I) Job Purpose
The Youth Team Leader provides systematic and ongoing planning, coordination, and supervision of assigned team staff. This position also provides direct care services with youth and caregivers. The Youth Team Leader ensures program adherence to all agency and program policies, guidelines, procedures, and responsibilities.
II) Essential Functions
A) Take lead in hiring and supervising team staff while providing leadership and ensuring professional accountability and productivity for staff.

Supervise team staff in assigned clinic(s) and community-based locations.
Monitor consumer services for quality and clinical appropriateness.
Ensure that team is adequately staffed to meet established program objectives.
Assign and actively manages caseloads.
Model behaviors that promote team-based services and treatment.
Adhere to all agency and program guidelines and procedures and ensure staff understanding and adherence.
Establish and maintain a team approach in treatment provision and meeting programmatic needs and demands.
Encourage and participate in exchange of information on youth issues, crisis management, and problem resolution among staff.
Participate in and assist with leading staff meetings at intervals congruent to clinical needs of the team in order to disseminate information and provide consultation.
Assist in developing and modifying goals, policies, and procedures to meet specific needs of the team.
Ensure team is operating in compliance with community standards, billing requirements, and agency and program policies and procedures.
Ensure team is adequately staffed to handle all crises and urgent issues for assigned youth.
Provide individual consultation to each team staff person in order to identify barriers to services, provide case consultation, and assist with problem-solving related to clinical service.
Contribute to the development of performance standards for staff and monitor performance. Provide feedback and support to staff regarding performance. Identify and carry out corrective action as needed.
Complete performance appraisals for each staff person annually or as directed by agency and/or Director.

B) Serve as liaison with inter- and intra-agency programs.

Maintain and foster positive cooperative relationships and communication between inter- and intra-agency programs and team staff.
Ensure that program is accessible for referrals to services. Serve as liaison in resolving coordination/referral problems.
Provide for staff work schedule(s) such that program coverage needs are met 100% of the time.
Notify the Director and/or appropriate agency Administrator within 24 hours of special issues, significant barriers, and adverse events in the community likely to affect services.
Respond to requests for information in accordance with agency and program policies and procedures.
Attend inter- and intra-agency meetings as requested.
Report critical and non-critical events to the Director and Risk Management consistent with agency and program policies and procedures.
Carry assigned mobile phone and ensure that equipment is properly charged and operational.

C) Provide individualized, trauma-informed, culturally, and linguistically competent services based on evidence-based practices for youth and caregivers in youth services

Make contact with youth and caregivers to encourage and maintain involvement at a frequency based on the strengths, needs, abilities, and preferences of the individual as identified on the wellness plan and agreed upon by the youth, caregivers, and multidisciplinary team
Deliver evidenced-based treatment of mental, emotional, behavioral, and substance use disorders and/or conditions. This includes (but not limited to) mental illness, emotional disturbances, substance use, and co-occurring disorders.
Direct care services will be provided to youth and families face-to-face and in accordance with agency and program standards.
Services are provided in locations that meet the needs and preferences of the individuals. Such locations may include residences, schools, correctional settings, community centers, shelters, day habilitation centers, hospitals, medical facilities, and/or other service sites.
Maintain current health records, which includes (but not limited to): registration, benefits assessment, financial assessment, privacy documentation, consents, assessments, wellness planning, documentation of all direct, indirect, and attempted contacts with individuals involved in behavioral health services (utilizing collaborative documentation when individual is present and as appropriate), and communications and records provided by outside providers.
Provide coordination of care with agency and community providers.

D) Develop and submit special reports and assignments.

Complete reports and/or assignments as requested by the Director.
Develop efficient process for data collection, minimizing impact on program operations.
Submit all reports as scheduled and/or requested by Director.
Assist in identifying barriers to staff performance and clinical expertise when appropriate.
Present reports and other information to inter- and intra-agency groups as designated by Director.

E) Persistently maintain a culturally competent and culturally sensitive environment in work locations for all youth and caregiver engagement at all times. Staff member is expected to behave in a culturally sensitive manner in all professional environments, regardless of location.
F) Work as a Member of Behavioral Health Team

Attend all scheduled staff meetings.
Provide input and engage in problem solving with other team members.
Serve on provider services/agency workgroups as agreed upon by Director.
Maintain positive, professional attitude in interactions with other team members 100% of the time.
Adopt trauma-informed practices into every level of operations and encounters through active involvement in the MHMR Tarrant Trauma-Informed Care transformation.

G) Complete and maintain all required training

Complete all agency-required training within first 60 days of hire.
Complete all required MHMR Tarrant training and maintain current training status.
Complete all required program-specific and role-specific training and maintain current training status.
Complete training updates as specified under agency procedures and maintains current status.
Obtain prior authorization from Director for special training at least two weeks in advance

IV) Internal & External Customer ServiceThis position requires extensive internal and external contacts with individuals served, family members, significant others, inter- and intra-agency staff, service providers, and practitioners. The employee will accomplish this with exceptional written and verbal skills. Contacts may include, but not limited to, answering phone calls and emails within one business day and conducting self professionally in all interactions.
V) Travel
This position requires transporting clients and driving between locations as an essential function of work. Employees are required to have a license in good standing and be able to be covered by agency liability insurance. If personal vehicle is utilized for work travel, then maintaining personal auto insurance coverage is required.
