Position : Social Services Director
Job Description : Bridger Rehab and Care Center, a 100+ skilled nursing facility located in Bozeman, MT, has an opening for someone to fill role of Social Services Director on our management team.This full-time position is critical to the successful resident/patient care at our facility and serves our community with primary duties such as admissions and assessments along with care conferences with residents and families.This position also educates and informs new residents and prospective residents of services offered at our skilled nursing facility.In this position, you identify and provide for each resident’s medical-related social services needs and promote each resident’s potential during their time with us. You’ll evaluate each resident and formulate a written plan of care; review and revise that plan as necessary; and refer residents and families to appropriate community resources. When appropriate, you prepare residents for discharge.In addition to directing the social services at the facility, this position will also assist with activities, outreach, marketing and community relations.A successful candidate will be able to demonstrate supervisory skills, creativity and strong leadership skills. Must be able to work in a collaborative way with the management team and support staff and be able to generate and maintain a resident-centered approach to the delivery of care.Qualifications: BA/BS preferred.At least one year of Social Services experience, preferably in a skilled nursing facility.At least one year of management or supervisory experience preferred.Excellent communication skills, both verbal and written.Excellent computer skills and attention to detail.Excellent time management ability.Self starterExcellent references and ability to clear a background check.EOE AAINDLPJob Type: Full-timePay: $20.00 – $22.00 per hourBenefits:401(k)Dental insuranceHealth insuranceHealth savings accountLife insurancePaid time offReferral programVision insuranceSchedule:Monday to FridayAbility to commute/relocate:Bozeman, MT: Reliably commute or planning to relocate before starting work (Preferred)Education:Bachelor’s (Preferred)Experience:Social Service: 1 year (Preferred)medical facility: 1 year (Preferred)License/Certification:Driver’s License (Preferred)Work Location: One location
