Position : BIRTH CERTIFICATE COORDINATOR – MEDICAL RECORDS

Job Description : Primary Duties and ResponsibilitiesThe Birth Certificate Coordinator is responsible for obtaining the necessary data elements for completion of the birth certificate as required by State Law. This role is also responsible for the application process for social security number and entering birth information into the Electronic Birth Registration System (EBRS). Completes and obtains signatures for other state specific programs including completing paternity forms. Processes affidavits for amendments to Birth Certificates. All documentation is to be completed and submitted according to State guidance and the Hoag Birth Certificate Policy and Procedure. Works closely with the Obstetrical nursing Leadership when carrying out duties. Performs other duties as assigned.Education and ExperienceRequired:

High school diploma or equivalent.
Computer skills.

Preferred:

Previous medical record and birth certificate experience.
Bilingual.
