Job Description : Responsibilities
Serve as a Community Health Worker for the BP REACH (Blood Pressure Disparities Reduction, Equity and Access among safety net patients with cardiovascular health risk) Project. Major duties will include; contacting and scheduling study participants for visits; following study protocols and making decisions based on outlined protocol; coordinating participant’s education with medical team; maintaining electronic databases; preparing and maintaining supplies for participant education use; conducting home visits to study participants home. You must have access to reliable transportation to conduct home visits and facilitate the health education in community settings.

Qualifications

Required:

Ability to provide health education for study participants.
Ability to use computer, mobile devices, and software provided for patient care.
Ability to perform duties and tasks independently with minimal supervision.
Ability to take initiative and follow through independently on projects.
Ability to effectively and compassionately communicate with patients to coordinate care.
Ability to maintain, follow up and complete tasks related to participant care on a timely basis.
Demonstrated cooperative behavior and positive-problem solving and conflict resolution skills.
Available to work dependable, flexible shift.
Complete and pass background check by Los Angeles County Department of Health Services (LAC DHS).
Excellent written and verbal bilingual communication skills (English/Spanish).
