Job Description : Description:
Kannact Registered Nurse Health Coaches engage with participants to better manage their chronic health conditions through risk factor identification, physiological parameters, goal setting, education, and emotional support. The coach engagement involves a knowledge of the participants’ disease state, SMART goal setting, and motivational interviewing techniques. Registered Nurse Health Coaches should be able to draw from clinical experience and medical knowledge to provide a heightened level of remote care to Kannact participants with the more complex cases. This position is mostly remote but we are looking for candidates within the Albany, Oregon who are able to make frequent visits to our headquarters.
. Requirements:
Scheduled health coaching calls to create a trusted relationship with participants resulting in a participant developed action plan adhering to clinical guidelinesUse of motivational interviewing techniques to develop actionable goals that result in long term behavior changeRespond to and document all communication with participants in a timely and accurate mannerMaintain participant health data privacy through HIPPA complianceAssist participants in troubleshooting devices provided by KannactUnderstanding of when to escalate issues to the Kannact medical or pharmaceutical teamsAbility to convey complex medical information to the participant in understandable language that enables effective change
Traits:Comfortable managing a large caseload in a remote settingPassion to make an impact on the health of each participantEnjoys consistent personal interactionWorks well within a team environmentQuick learner with high work ethicManages time well
Required Education and Experience:Current state RN level licensureExperience with patient interaction with strong bedside mannerKnowledge of chronic conditions, related medications, and care of these conditions
Preferred Education and Experience:Direct clinical experience working with patients with chronic diseasesExperience educating and motivating behavior change in patientsBachelor’s degreePublic Health experience
