Job Description : In accordance with the Agency’s mission statement, policies and procedures, professional Code of Ethics, State of Michigan licensing and COA standards; the Intake Specialist is responsible for providing intake services which will accurately assess the programmatic needs, and subsequently refer youth and families to the most appropriate agency program.PRIMARY DUTIES AND RESPONSIBILITIES: Plans and organizes work to meet all job requirements according to agency and COA standards, licensing, and funding requirements. Regularly prepare for and participates in supervision, management team and other agency meetings. Works cooperatively and in collaboration with Vista Maria staff and representatives of other agencies. Works with various branches of DHHS, the courts, and community agencies to facilitate referrals to Vista Maria’s programs and provide clients with assistance as needed for external agency referrals. Ensures all required admission documentation is completed promptly which will ensure accurate and timely client program assignment and/or referrals. Communicates clear and timely information regarding the intake, eligibility, follow-up processes and procedures related to specific client needs. Completes all required paperwork, (e.g., case records, periodic reports, and service activities, census information). Uses supervision, educational and training opportunities to ensure continual professional competence. Participates in continuous quality improvement efforts. Enters pertinent client information into databases such as STARs, Extended Reach and MiSACWIS as required by policy and procedure. Conducts community presentations to referrals sources for all Vista Maria foster care, residential and housing programs and supports other marketing initiatives as directed. Conduct weekly outreach to referral sources regarding placement availability for all programs (i.e. DHHS, CMO, courts and community agencies). Responsible for rotating on-call coverage for emergency placement needs. Demonstrates personal integrity and professional demeanor in accordance with the ethics of the agency and profession. Conducts client interviews and matches accepted clients with appropriate foster families with input from the team. Supports foster parents, and foster child with transition into new foster home and visits foster home within 72 hours. Sends referrals as appropriate to Shepherd hall dorm team for consideration and acceptance. For youth accepted for dorm apartments, completes required documentation and case file per admission procedures and visits SIL youth within 72 hours of placement. Performs all other duties as required or requested.QUALIFICATIONS: Minimum Required: Bachelor’s Degree in Human Services from an accredited school. Two (2) years’ experience in a social service program.. Valid Michigan Driver’s license without relevant restrictions. Demonstrated computer literacy skills as required. Demonstrated ability to interact positively in a culturally diverse environment. Demonstrated understanding of social services/social work as a professional practice in general. Ability to be flexible in prioritizing daily responsibilities and flexible in work schedule Ability to problem-solve crisis situations and direct after hour emergency placements.Preferred: More than two (2) years’ experience in a social services program One (1) year or more experience in intake Demonstrated advanced computer skills Demonstrated competency in public speaking and marketing outreach.Job Type: Full-timePay: From $40,000.00 per yearBenefits:Dental insuranceFlexible scheduleHealth insuranceLife insurancePaid time offReferral programVision insuranceSchedule:8 hour shiftMonday to FridayOn callEducation:Bachelor’s (Required)Experience:Human services: 2 years (Required)License/Certification:Driver’s License (Required)Work Location: One location
