Job Description : Description:
Supervises daily activities in the Homes. Assures compliance of the home to all state/Federal rules and guidelines and regulations, MORC, MDHHS, Perfecting Lifestyles rules and policies. Promotes a positive, cooperative relationship with day programs. Supervises all Direct Support Professionals assigned to the Homes. Works specified hours of direct care, observations, Training’s and meetings per the needs of the home/agency. Assists Individuals in achieving and maintaining their individual health, hygiene, occupational, social, and emotional goals, While assisting Individuals employees may encounter intense behaviors and physical aggression. Employees are expected to approach the Individuals we serve with a high level of professionalism that puts first respect, Gentle Teaching and dignity of our residents. Monitors assigned sites during specified times that will fluctuate per needs of the sites. Team Leads will provides to Individuals assistance in the following areas:
a) Money management
b) Nutrition
c) Leisure activities including social, recreational, and community access
d) Emergency situations
e) Health care
f) Other areas of concern as designated by the Interdisciplinary Team.
Supervises the implementation and documentation of Active Treatment including ipos, Behavior Management/Support Programs, and medication dispensation. Verbally reports, investigates, and writes written reports, and follows up on all aspects of Individual abuse/neglect cases within prescribed timeframes. Communicates with families or Advocates on a regular basis regarding Individual’s needs and progress. Communicates with support services in a manner that assures Individuals are receiving optimal services on an ongoing basis. Works with Area Directors and human resources to ensure coverage of all shifts. Maintains accurate paperwork, including (but not limited to):
a) Assistance with staff schedules and call-offs
b) Narratives
c) Ipos data collection cls logs, calendars
d) Behavior data e) Assistance with timesheets
g) All month-end paperwork From time to time, the employee will be required to perform additional tasks and duties as required by the employer.

The Team Leader is a member of the Perfecting Lifestyles LLC Home Help Services and Interdisciplinary teams. Team Leaders assist and supervise the day-to-day activities of the in Home programs of specific sites, including staff selection, training, supervision, and paperwork. The Team Leader is also responsible for ensuring the coordination of overall house operations.
