Build and maintains effective working relationship with integrated family medicine residency, community partners, and other RiverStone Health service lines. Ensure continuous compliance with all 330 (health center) grant requirements, National Committee on Quality Assurance (NCQA) Patient Centered Medical Home requirements and other current standards of outpatient primary care and safety. Maintain current policies, procedures and workflows to ensure consistency, accountability and adherence to standards of care and safety. Facilitate effective and efficient health care operations, including but not limited to quality of care, financial stability, staff engagement, environment of care, risk management and safety. Lead and mentor staff to foster engagement in the mission of RiverStone Health.ESSENTIAL KNOWLEDGE, SKILLS AND RESPONSIBILITIES: Oversee service development that is responsive to community need and consistent with the Health Center Needs Assessment, including evaluating clinic performance, effectiveness and efficiency. Establish clinic goals and objectives to execute planning.Champion the Patient Centered Medical Home model of care and incorporate the associated tenets into ongoing quality improvement initiatives.Lead and mentor staff, including hiring decisions, orientation, staff development, evaluations and feedback, corrective action, promotions, discharge of personnel, and ensuring accurate and timely recordkeeping. Participate in and support the Credentialing and Privileging policies and procedures, consistent with 330 grant requirements.Ensure appropriate staff coverage through effective and consistent scheduling processes.Oversee quality improvement work in the main clinic, in coordination with Director of Quality and Innovation, to ensure staff engagement in the process and clarity of objectives, as well as progress toward improvements.Actively participate in the fiscal year budgeting, including but not limited to productivity projections, revenue and expense projections as well as capital budgeting. Review and analyze monthly financial reports and make operational adjustments as necessary.Review and analyze monthly utilization data, including but not limited to productivity of providers and staff, patient volume etc. and make operational decisions and recommendations as appropriate, including but not limited to opening/closing panels, additional coaching/training, workflow changes or other interventions. Ensure utilization of dashboards and reports by staff and provider to monitor progress toward goals and objectives.Oversee and identify staff training and development needs and opportunities. Facilitate content experts or trainers and facilitate process for training and documentation of content and attendance.Oversee and facilitate effective staff meetings and opportunities for communication, team building and learning.Maintain visibility and accessibility for staff and provider to encourage open and transparent communication, constructive feedback and problem solving.Build and maintain relationships with community partners as appropriate to ensure awareness of and coordination of services.Build and maintain effective relationships with other RiverStone Health service lines to ensure awareness and coordination of services and to foster confidence in the clinic services and seamless care to patients.Incorporate consideration of the patient experience in quality improvement activities; addresses concerns, complaints and suggestions to resolve issues and make operational improvements in a timely manner. Evaluate requests for special discounts and waivers and make appropriate decisions, with timely documentation of the request and decision.Actively participate in risk management activities, including but not limited to ensuring staff adherence to RL6 reporting expectations, incident investigation and follow-up, facilitating compliance with dismissal from care decisions and participating in that process with decision-making, actively engaging in Root Cause Analyses and Ad Hoc Case reviews as appropriate, to ensure safety in the environment of care and in the delivery of services.Oversee and ensure active participation by the main clinic in Infection Control Committee, Pharmacy and Therapeutics Committee, 340B Oversight Committee, Quality Improvement Committee.Develop and maintain current policies, procedures and workflows that are reflective of current processes and requirements.Provides direct supervision clinic management team to efficiently and effectively coordinate patient care. Motivate, coach, develop and evaluate direct reports.Develop and implement clinical programs designed to improve processes, healthcare delivery, and ultimately health outcomes for patients of the Federally Qualitied Health Center (FQHC)Identify opportunities and unmet needs in the community. Develop workflows and services to meet those needs, consistent with HRSA scope of project and grant requirements.Actively participate as a member of the CHC Leadership Team.Other duties as assigned.QUALIFICATIONS to perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
