Position : DDD COORDINATOR
Job Description : The DDD Coordinator performs care coordination and placement activities for children involved with the Department of Child Safety (DCS), the Department of Developmental Disability (DDD) and the DDD Arizona Long Term Care System (ALTCS).
Job Duties:
Performs care coordination and placement activities for children involved with DCS, DDD, and DDD ALTCS who have complex physical and health care presentations; leads efforts to garner appropriate caregiver to child match and ensure necessary supports are in place to meet the needs of the child and caregiver.
Supports DCS Specialists and other stakeholders navigate the DDD eligibility processes, including the tracking of referrals to DDD; assists DCS Specialists and DCS Placement Administration to garner necessary medical documentation from health care professionals and other required information when needed; tracks and reconciles enrollment of children and youth determined eligible for DDD services.
Develops and maintains professional relationships among system stakeholders, including DCS CHP System of Care staff with efforts focused toward identifying child-specific and system oriented enhancements to improve health care to children served by DCS and DDD.
Participates in DCS’ performance management activities that support positive health care and individualized plan outcomes for children and youth. Maintains and/or manages health care and child-welfare data, develops and distributes reports as appropriate to staff and leadership.
Maintains up-to-date knowledge of DCS, DCS CHP and DES DDD policies and procedures and applicable federal and state laws.
Participates in activities beyond regular work hours on a rotational basis; handle incoming telephonic inquiries from members, providers, caregivers and DCS staff during off hours.
Knowledge, Skills & Abilities (KSAs):
Knowledge of:
Applicable laws and regulations, government (Medicare, Medicaid, SSI, and SSDI) DCS and DDD Policy and Contractors Operations Manuals.
New regulations on federal, state and local levels as well as practice guidelines and standards of practice.
Ability to:
Assess, determine, and evaluate appropriate disposition and make independent judgments based on critical thinking skills and expertise.
Demonstrate professional behavior, promote cooperation and team building.
Communicate effectively and professionally using all modalities (i.e. technology, written letter, and verbal) with both providers and caregivers in a way that is both clear and concise.
Maintain and manage to caseload working independently with minimal of direction, anticipate and organize work flow, work with a high volume caseload, deal effectively with rapidly changing priorities and follow through on responsibilities.
Selective Preference(s):
Bachelors degree in counseling, social work, psychology, or related field; Two years of health care advocacy through the child and family team process, developmental or physical health providers or systems systems including managed care or child welfare system required.
Pre-Employment Requirements:
In-state travel is required. Requires possession of and ability to retain a current, valid state-issued driver license appropriate to the assignment. Employees who drive on state business are subject to driver license record check, must maintain acceptable driving record and must complete any required driver training (see Arizona Administrative Code R2-10-207.12). Employees may be required to use their own transportation as well as maintain valid motor vehicle insurance and current Arizona vehicle registration. Mileage may be reimbursed, if applicable. For valid Arizona Driver’s License holder, a copy of your three (3) year Motor Vehicle Report will be required. The ability to secure and maintain an Arizona Fingerprint Clearance Card; and the ability to secure and maintain clearance from the DCS Central Registry.

Retirement:
Positions in this classification participate in the Arizona State Retirement System (ASRS). Enrollment eligibility will become effective after 27 weeks of employment.
