Main duties of the job

The successful candidate must be prepared to work flexibly and sometimes outside core hours of 9-5. The team are newly formed team with amazing leadership and very excited to welcome a new member of the team aboard. The successful candidature will also be required to provide nursing interventions to meet the complex needs of a young person in school supporting the young person on transport to and from school

There will be lots of opportunities for learning new skills; the successful candidate must be comfortable in working in patients homes, liaising with other hospitals and preparing care plans for children in the community. There will also be opportunities to work directly with complex needs children within the school setting.

KEY DUTIES

Support with training in line with the identified package of care for the CCG funded child/young person.
Plan and deliver individual programmes of specific care for children within a range of care and educational settings, who have a variety of healthcare needs in a competent manner that recognises own limitations and seeks appropriate supervision.
Possess advanced clinical skills to closely monitor the child’s progress and improvement and to detect deterioration, promptly following the agreed care pathway to medical reassessment.
Deliver specific health education, health promotion and illness prevention advice to fellow service providers and children / families.
To respect family’s cultural, spiritual and religious practices and work in partnership with families in promoting parent participation and child / family centred care.
Participate in the proactive planning process for the transitional period, prior to young people transferring into adult care services.
To manage own caseload and ensure effective management and time keeping skills.

CLINICAL & PROFESSIONAL RESPONSIBILITIES
To adhere to professional standards in accordance with the Nursing and Midwifery Council’s (NMC) standards of Professional Practice and Code of Professional Conduct.
Undertake mandatory training sessions and update own professional clinical knowledge and skills, also reviewing and maintaining own clinical competencies and assist the service and clinical coordinator in the ongoing development of fellow team members.
Participate in the identification of personal professional and team development objectives through performance management systems.
Actively seek support systems that promote safe, ethical and professional clinical practice.
Participate in a clinical supervision process, as part of a personal development plan, in order to meet the Post Registration Education and Practice (PREP) requirements to maintain registration.
Ensure that verbal communications and written documentation are maintained and are clear, relevant and concise, in accordance with Nursing and Midwifery Council’s (NMC) Standard’s of Records and Record Keeping and Trust policy.

ADMINISTRATIVE RESPONSIBILITIES

Employees of the Trust must comply with the provisions of the Data Protection Act 1998. The postholder must not; either during the course of their employment, or following termination of their employment, disclose any information relating to service users or employees, or of the lawful business practices, of the organisation.
The post holder will be required, when and where appropriate to the role, to comply with the processing of requests under the Freedom of Information Act 2000
The post holder must comply with Trust policies that protect the information assets of the organisation from unauthorised disclosure, modification, destruction, inappropriate access or use and must pay due regard to the requirements of the NHS Confidentiality Code of Practice.
The post holder will be responsible for maintaining the clinical and or corporate records that fall within the remit of this role to the standards in Trust records management policies, and data quality processes and standards.

TEACHING & TRAINING RESPONSBILITIES
Facilitate the training and ongoing support of children / families / carers and all professional colleagues, according to their level of knowledge, ability and willingness to participate in care delivery, including the assessment of their knowledge and competence.
Participate in promoting professional development within the team and fellow healthcare professionals, in order to develop skills and competencies, in accordance with the Knowledge Skills Framework (KSF).
Assist in the support, training and assessment of student nurses, ensuring their learning outcomes link theory to clinical practice.
Participate in developing networking links with colleagues within all relevant health, education and social care colleagues.
Within the team, share new skills, knowledge and expertise, which is proven best practice and evidence based.
Participate in the development and evaluation of specific training information and material’s, in order to disseminate and promote evidence based care to all relevant colleagues and families.

LINE MANAGEMENT/SUPERVISORY RESPONSIBILITIES
Receive and assess any referrals and be responsible for the planning and delivery of high quality, evidence based care for children with a variety of healthcare needs. Liaise with service and clinical coordinator as appropriate.
Participate within staff performance reviews and the formation of personal and team development plans.
Participate in the development and review of the services Care group and organisations policies procedures and guidelines
To be responsible for the most effective and efficient use of resources and equipment and to rapidly identify any replacement needs and resource issues.
Participate in the continuing development of induction, orientation and preceptorship programmes for all permanent team members, bank staff and student nurses.
To be responsible for managing own daily workload and planning, prioritising and delivering care effectively, whilst identifying and managing any risks to the quality of care provided.
