The Care Co-ordination Centre are looking to recruit highly motivated Health Care Support Workers to join our rapidly growing team.
The role will provide support to the nursing team working collaboratively with members of the multi-disciplinary team and contributing positively to the delivery of high quality compassionate care.
The successful candidate should have excellent communication skills and demonstrate delivery of high standards for patient care together with a drive and passion for improving the way healthcare is provided. They must be flexible, a good team player and have the ability to work under pressure.
The Care Coordination Centre is the central hub for referrals and contacts into the Adult Community Nursing Services where we triage and signpost our referrers and patients to the most appropriate service to meet their need. We utilise up to date technology to allow us to complete referrals and monitor our patients remotely, therefore you must be confident using telephones, IT systems, video calls and web based application to provide patient care.
