Position : Emergency Services Case Manager

Job Description : This Emergency Services Case Manager position is for our SafetyLink Program. A Bachelors Degree in social work, psychology, human services, education, or related field, and a minimum of 12 months of verifiable direct case management experience in a closely related field is preferred. Knowledge of local resources and HMIS knowledge preferred. Dependable transportation and a Nevada Drivers license are required as this position involves local travel to client homes and hotel/motel shelter locations. All work mileage is reimbursed at state determined rates.
Provide case management and family support services to a caseload of 20-25 cases of families in a shelter environment. Responsibilities are varied and will include working with families in crisis and experiencing homelessness. This position places families in an off site hotel/motel shelter and provides them with wrap around services which will help stabilize the household. This position reports directly to the Family Resource Center Supervisor.
Other responsibilities will include:
-Meet with clients in office, in their homes, over the phone or over video call to collect information and complete rental assistance applications
-Keep accurate and updated notes regarding the assistance provided to each client
-Entry into HMIS and eLogic databases.
-Ability to maintain confidentiality with all client data
-Basic computer skills including excel, Microsoft word, Outlook, data entry, scanning and uploading documents.
Job Type: Full-time
Pay: $42,000.00 – $48,000.00 per year
