Job Description : Description:
Program Management
Directs overall implementation and management of health equity program and activities
Ensures compliance with CDC/DPHHS grant rules and reporting
Monitors and reports on appropriate use of grant funds
Serves as a liaison internally and with organizations using program services to monitor and evaluate ongoing needs and effectiveness of program
Outreach and Engagement
Develops and maintains collaborative partnerships internally and with other community organizations including local mental health advisory boards, healthy community coalitions, advisory groups for public health departments or local health care organizations
Engage community partners to implement health equity strategies and plans
Increase collaboration among the community health assessments and plans developed by CAHs, CHCs, and PH
Work with MORH/AHEC Health Equity Coordinator, Community Planning Coordinator, local/regional/state health equity councils to boost meaningful utilization of Montana Connect to coordinate care and address social determinants of health
Develop new partners in underserved/rural communities to promote equitable access and participation in AHEC programming
Data Collection and Analysis
Develop plans based on the results and findings of COVID after action reports
Build upon health equity focused strategies established during COVID by local public health and healthcare coalitions, groups, boards, and organizations
Work with local public health and healthcare coalitions, groups, boards, and organizations to improve skills for utilization of data for local public health
Lead consultant in the development of a Healthy Places Index for Montana to help public health and healthcare address social determinants of health
Conduct audit of existing AHEC program data and create plan to address areas of improvement, grow equitable programming, and increase participation of rural and underserved communities
Training Coordination
Manage logistics for in-person and virtual training and education programs
Recruit trainers for behavioral health trainings, provide trainers data collection documents, develop forms to collect participant and facility information, respond to trainer inquiries, collect and upload documents to Box, work with accounting department on trainer reimbursement
Recommends changes in program content, location, venue, trainer and/or other program elements
. Requirements:
Bachelor’s degree in a relevant field
Experience in public health, healthcare, and/or education preferred
Must have a valid Montana driver’s license and complete a background check
Knowledge of public health and healthcare systems and programs.
Knowledge of social determinants of health and health equity.
Excellent written and oral communications skills.
Excellent organizational skills.
Demonstrated ability to coordinate and facilitate large groups.
Demonstrated ability to multi-task and manage multiple projects and deadlines.
Demonstrated ability to problem-solve in a positive and constructive manner.
Demonstrated ability to work both independently and in a team environment.
Experience working with individuals of diverse backgrounds.
Experience using social networking platforms, including Zoom and Teams.
Proficiency with computer skills, including MS Office.
Demonstrated ability to collect and analyze data using tools.
Travel: 40% travel time, including some overnight travel will be required.
