Description

Mission:

The Mission of St. PJ’s Children’s Home is to be a safe and loving refuge for children in crisis and to accompany them on their journey to healing and wholeness, breaking the cycle of abuse and neglect one child at a time.

Summary:

The case manager provides short-term case management services to children who have been removed from their home by Child Protective Services and/or those children, who for specific reasons, are unable to live at home. This is accomplished by assessing the needs of a child. Focusing on the individual as well as the ”system” in which s/he lives; developing comprehensive service plans, making and accepting referrals on behalf of assigned children, and making decisions on behalf of the agency regarding medical/dental/vision treatment and educational services for the children. The case manager will work with other case managers, community resources and other community-based providers/representatives; the case manager arranges, coordinates, monitors, evaluates and advocates for a “package” of multiple services to meet a child’s specific needs.

Case Manager Tiers:

Incoming case managers will be hired at the CM I level, with the opportunity to increase to CM II and CM III level based on time on the job and other factors.

CM II qualifications:

At least 6 months or more in current position.
Ability to accept a full case load of 15+ children.
Ability to be a part of the on-call rotation; on-call lasts one week at a time.
Attend/complete at least 10 hours of training on special topics beyond the required annual training hours.
Provides at least 6 hours of staff training a year, as requested.

CM III qualifications:

At least 1 year or more in current position.
Must meet all CM II requirements.
RCYCP certified, when offered by facility.
Assists in training incoming case managers.
Provides at least 12 hours of staff training a year, as requested.

Position Responsibilities:

Serves as the educational decision maker and medical consenter for the children on their assigned case load, as assigned by the Texas Department of Family and Protective Services.
Signature authority for children’s medical treatment and educational decisions.
Ensure all children’s records, including medical and educational files are current with appropriate documentation included, such as admission assessments, diagnostic assessment and treatment plans, therapy notes, plans of service/reviews; medical, dental, psychiatric and psychological exams and reports; current court orders and appropriate consents and agreements; report cards, educational testing, discipline referrals, etc.Develop and facilitate implementation of admission assessments, diagnostic assessment and treatment plans to ensure quality care and compliance with TDFPS (Texas Department of Family and Protective Services) Minimum Standards, and YFT (Youth for Tomorrow) Program Standards.Demonstrate proficiency in knowledge of psychotropic medication, SAMA, CPR/First Aid and child abuse reporting procedures.Actively participate in professional development such as attending at least 20 hours of continuing education as required by appropriate licensing board.
Effectively and efficiently collaborate with and communicate information regarding children on his/her caseload to team members including Team Leaders, direct care staff, therapist, teachers, psychiatrists, parents/guardians, attorney ad litem, etc.;
Adopt a proactive stance on behalf of children on his/her caseload. Ensure all children served have appropriate advocacy support, internally and externally.
Demonstrate proficiency in children’s psychological disorders, particularly those common in the abused/neglected child.
Attendance and successful completion of a 10-day New Employee Orientation is mandated for the position. The inability to meet this requirement will conclude employment with the Agency.

Requirements

Minimum Qualifications:

Education

Bachelor’s degree in social sciences or related field.

It is preferred the case manager have two years’ experience.
Ability to be credentialed by Star Health and TDFPS as medical/education decision maker.
License and Credentials for:

Reliable transportation
Valid driver license
Valid vehicle insurance

Minimum Knowledge and Skills:

Minimum of 2 years’ experience in case management or in providing services to abused/neglected children and their families and/or experience in residential child care, separation, attachment and permanency issues is preferred.
Computer literate with working knowledge of Microsoft Office (Microsoft Word, Excel) and a variety of online database systems, managing files and records, designing forms and other office procedures and terminology;
Strong analytical and organization skills and abilities;
Must be able to read and interpret policy, procedures and standards;
Must be detail oriented, organized, self-motivated, work well independently and on a team;
Must have good written and verbal skills;
Must have good critical thinking and problem solving skills;
Must be able to remain calm and composed in crisis situations;
