American Premier Hospice is looking for multiple Registered Nurse’s for home visits for our patients.
Job Summary:
The primary functions of our Hospice Registered Nurse are to administer skilled nursing care for hospice and home health patients either in their place of residence, a skilled nursing facility or an assisted living facility. The duties of the Hospice Registered Nurse are to assume responsibilities of the coordination of care between the patient, healthcare providers, team members, as well as their families as needed.
Responsibilities

Professional Management of the Patient and Family
Communication with Medical Professionals, the Home Health/Hospice Team, and staff.
Maintaining Accurate, Up-to-Date Records.
Training the Patient and Caregiver.
Elevating Necessary Matters to the Attention of the Patient Care Manager.
Able to properly diagnose the patient’s condition and track decline.
Has efficient and effective writing skills to write thorough notes on patient.
Regularly re-evaluates patient condition.
Evaluates and measures performance of home aides.
Works collaboratively with the interdisciplinary team to ensure positive patient outcome

Qualifications:

Current Registered Nurse license in state of Arizona.
Two (2) years’ experience in a hospice and/or related setting preferred.
Must be CPR certified.
Valid State Driver’s license
Current Fingerprint Clearance Card
