POSITION SUMMARY
The Patient Support Specialist is responsible for assisting in patient care by helping compile documents and records prior to the patient’s appointments. This individual serves as an advocate for patient needs by coordinating care to minimize the wait for services and avoid delays in access to care.
Essential Functions and Job Responsibilities:

Perform outreach to assigned patients and assist in the coordination of care
Collect patient forms and ensure they are completed properly
Obtain insurance documents and photo IDs
Upload form, documents, and IDs into assigned electronic medical record (EMR) prior to
patient’s appointment
Notify providers of cancelled appointments due to missing forms, documents, and IDs to
allow for other appointments to be scheduled
Respond to patient phone calls, emails, and portal messages within timelines established by department
Provide de-escalation and basic coping skill interventions to retain the patient’s scheduled appointment
Participate in regular team meetings and peer review activities; participate in departmental and
organizational committees
Other duties as assigned

Education, Certification, and Experience Requirements

Bachelor’s degree in counseling, psychology, social work, or related field preferred.
Minimum one year experience in behavioral health or integrated medical office setting
Must obtain, or have, a valid Arizona Fingerprint Clearance card

Job Type: Full-time
Pay: From $18.00 per hour
