About you
We are hoping to welcome an approachable and knowledgeable Community Based Dementia Adviser to work across the local community. This is an amazing opportunity to have a rewarding career providing support and guidance to people with dementia and their carers. We strive to help people to maintain independence, improving their sense of well-being, and putting people in more control of their own lives. We promote a person-centred service so that our support is unique to each individual, based upon their own aspirations and needs.
You will need:
Great organisation and communication skills
A non-judgmental approach to all people and situations
An understanding of dementia
To know the importance of client confidentially and collaborative working
Be able to travel independently within the service area.
Demonstrable experience within advice & guidance settings or training.
About the role
We are looking for applicants to provide support to people living with dementia and their carers helping to maintain their independence, improving their sense of well-being, and putting them in more control of their lives.
The successful applicant will provide information, support and will facilitate group courses designed specifically for Carers and people with dementia across East Sussex
You will be:
Creating and running groups to support people affected by dementia.
Advertising the courses and make sure everyone knows the details.
Taking feedback from people who attend to keep improving what we are providing.
Guiding people to other services if needs be.
Welcoming all group members and help them feel at ease so that all are able to participate equally, as they wish.
Preparing written feedback for line manager on both individual and group needs with the aim of improving the group and the experience of people attending.
Managing referrals, assisting clients in assessing their information and support needs.
Maintaining and managing a caseload of clients who are affected by dementia be by phone, email, letter, or face to face.
